2008 FORMS
After registering on line you will need to fill out the correct information and medical forms in order to attend camp. These forms need to be filled out prior to camp and be sent at least two weeks before the camp session. Forms will be mailed to you when you register. Please review the following list of camp session types and the forms that are needed for each session. You will need Adobe Acrobat Reader to view and print these electronic forms. If you are missing a form, you may print it out and use it. All persons attending camp will also need to fill out an American Camping Association Health History form.
